Parents please download, print and complete the following forms when directed to do so by your child's school. If you have any questions or problems completing the forms please contact the office of your child's school and ask to speak with the school nurse.

Parent Notice

The Mississippi Board of Nursing informed all school nurses that a change is required in how we handle the administration of over the counter medications (OTC) while at school. Medications include, but not limited to, Tylenol, Ibuprofen, Mylanta/Maalox, Tums, cough drops, throat lozenges, etc... In order for us to administer OTC meds we are now required to have an order form signed by the students provider (MD, DO, NP, PA) giving permission for those medications to be given at school. IF YOU WOULD LIKE YOUR CHILD TO HAVE OVER THE COUNTER MEDICATION AVAILABLE AT SCHOOL, YOU WILL NEED TO HAVE THEIR PROVIDER SIGN THE FOLLOWING ORDERS AND RETURN THEM TO THE SCHOOL NURSE. Students who are signed up for our school based clinic Southeast Mississippi Rural Health Initiative (SeMRHI) will be able to get the medication order form signed through the school clinic. Our onsite clinics will be available to students of the LCSD at NO COST. Our clinics are available to every student, no matter if they are insured or not. If you have a current primary provider, you do not have to change providers and could still see your child's primary doctor. Our clinics will be available for same day sick visits, medication refills and other medical needs. Your child will never receive care through our clinic without your permission, and you have the choice to be present at the visit and or participate via phone.
